WISE Program
This educational program offers six sessions that encompass life issues that seniors face. It is designed to help older adults celebrate healthy aging, make healthy lifestyle choices and avoid substance abuse. This science-based program includes activities, where fun and learning occur through interactive small and large group discussions on topics such as:
- the aging process
- benefits of this stage in life
- how to avoid problems with alcohol and prescription medications
- tools to feel move empowered about your health and the healthcare you receive
Take Control of Your Health
If you, or someone you know, have a chronic condition, the Take Control of Your Health (TCH) program can help you take charge of your life.
Some examples of chronic conditions include:
- diabetes
- arthritis
- high blood pressure
- heart disease
- chronic pain
- anxiety
- depression
- and many others
These conditions may cause pain, fatigue, isolation and emotional distress. Good health is soundness of body, mind and spirit. To live a healthy life with a chronic condition means working at overcoming the physical and emotional problems caused by the disease. It is good to control the disease instead of the disease controlling you!
This program is held for 2 hours each week over a six-week period. These workshops will help you:
- understand the problems caused by your condition
- manage your health condition
- learn activities for better general health
These workshops do not deal with each chronic disease, such as diabetes or high blood pressure, individually. However, you will learn:
- how to solve problems
- how to deal with emotions
- how to breathe better
- how to relax
- how to deal with pain and fatigue
- how to have healthy eating habits
- how to communicate with others
- how to use medications correctly
- how to manage depression
- how to work with your health care professional

Sessions: 6 two-hour sessions for six weeks
Target Audience: Adults with a chronic condition and/or their caregivers
